211 U.S. Collaborative

Trouble accessing healthcare is not unusual in the United States. That’s why providers who take into account social determinants of health (SDOH) have a better chance of engaging with patients who otherwise wouldn’t be able to access care. 

This innovative app created by the United Way of Wisconsin enables providers to connect their patients to critical healthcare services. It works by using a community resource database that matches complex service categories to SDOH.

AAP Pediatric Patient Education

According to the U.S. Bureau of Labor Statistics, there are nearly 33,500 pediatricians in America responsible for millions of patient visits annually. The American Academy of Pediatrics (AAP) offers this app that places the pediatric patient education content directly into pediatricians’ clinical workflow. It consists of more than 1,000 patient education handouts, covering everything from well-care checkups and vaccination support to acute care follow-up.

This AAP app enables pediatricians to assign handouts based on a patient’s problem list or search for education resources based on the patient’s needs. These resources are then added to the patient’s MyChart account for anytime digital access.

CoverMyMeds Prescription Price and Status

There are more than 20,000 prescription drug products approved for marketing in the U.S. Each year, more than four billion prescriptions are dispensed. Designed to enable patients to view the best price and pharmacy for their prescriptions from within their MyChart patient portal, the CoverMyMeds real-time price comparison tool also allows them to view prior authorization status on medications they’ve been prescribed.

This virtual care solution for mental health helps healthcare providers connect to the company’s platform through Epic EHR to deliver on-demand digital therapeutic care to patients. The goal of this app is to improve outcomes, increase access, scale support and reduce costs. 

SilverCloud is part of Amwell’s Comprehensive Behavioral Health Program, which provides healthcare organizations with the ability to offer virtual therapy and psychiatry, coaching and digital mental health programs. UnityPoint Health in Iowa was the first health system to integrate SilverCloud into its EHR.

Helpful

Designed for caregivers, this app was created by Helpful, a company whose goal is to make the lives of those caring for loved ones easier by automating and eliminating unnecessary steps. On the app, a patient’s benefits are automatically displayed in an accessible and easy-to-understand format. REDCap

Created by Vanderbilt University Medical Center, REDCap is a secure web application for building and managing online surveys and databases to collect data for clinical research studies. It provides automated export procedures for seamless data downloads to Excel and common statistical packages as well as a built-in project calendar, scheduling module, ad hoc reporting tools and other features. 

AJCC 8th and Future Editions Cancer Staging Forms

Publications from the American Joint Committee on Cancer (AJCC) are widely recognized as the authoritative guides for cancer staging information. This AJCC app provides clinicians with access to the latest and most comprehensive staging guidelines when diagnosing and planning treatment for a patient’s cancer. 

Glytec Glucommander
